How to Claim 

We’ve made it easy for SimpleCare members to claim medical expenses using our state of the art digital tools.

​If you have paid for out-patient treatment (e.g. a GP visit) and need to submit a claim for reimbursement, you can do this in one of two ways:​

  1. Via your secure online portfolio​.
  2. Via our smartphone App.​

Secure Online Portfolio

Our secure online portfolio enables members to quickly and easily submit claims online.

Simply go to the 'My Claims' page and complete the online form. Don’t forget to attach any receipts or diagnostic reports. ​

You will receive a notification to acknowledge receipt of your claim and you can track the status using the My Claims area of your secure online portfolio. ​

Smartphone App 

You can also submit a claim using our smartphone App for iPhone and Android, making it easier for you to manage your claims when on the move. ​ Download our smartphone App for iPhone and Android below.

Simply complete the online form and don’t forget to attach any receipts or diagnostic reports.​

You will receive a notification to acknowledge receipt of your claim and you can track the status using the My Claims area of your secure online portfolio. ​

In-Patient treatment​

If you need to be admitted to hospital for in-patient or day-patient treatment, you must contact us first to request treatment pre-authorisation. ​

We will then place a guarantee of payment with the medical provider so you do not need to pay and claim back. ​

If you've accessed eligible treatment within our out-patient direct billing network or we've placed a guarantee of payment for you, there's no need to do anything further. To find a medical provider in our network, either download our app, visit our User Guides page or contact your customer service team.

If you've had to pay for medical treatment and want to claim back your expenses, you can use the Now Health app to submit your claim.  Alternatively, find the membership member prefix that matches yours, and follow the steps below:

For customers with membership numbers starting AP, ME, EU, GL, EX, UK, SG, ID and BN, the following claims process apply:

All out-patient claims and in/day-patient claims under USD 500/EUR 400/GBP 300/SGD 650
For all out-patient claims and in/day-patient claims under USD 500/EUR 400/GBP 300/SGD 650 (per insured person, per medical condition, per period of cover), you just need to complete the front of the claim form and email it to us with your scanned receipt. If you prefer, you can send it by post or fax instead.

All in/day-patient claims over USD 500/EUR 400/GBP 300/SGD 650
For all in/day-patient claims over USD 500/EUR 400/GBP 300/SGD 650 (per insured person, per medical condition, per period of cover) complete the front of the claim form and ask your medical practitioner to complete the back. Then email it to us with your scanned receipts/diagnostic reports/discharge reports. If you prefer, you can send it by post or fax instead.

For customers with membership numbers starting CH, the following claims process applies:

Out-patient and in-patient/day-patient treatment under RMB 3,000 per medical condition
For all treatment under RMB 3,000 (per insured person, per medical condition, per period of cover), you just need to complete sections 1 and 2 of the claim form and email it to us with your scanned receipt. If you prefer, you can send it by post or fax instead.  Please sign your name on each medical expense receipt.

In-patient/day-patient treatment over RMB 3,000 per medical condition 
For all treatment over RMB 3,000 (per insured person, per medical condition, per period of cover) you need to complete sections 1 and 2 of the claim form and ask your medical practitioner to complete section 3. Then send it to us with your the original receipts/diagnostic reports/discharge reports.

Processed in five days
Providing we have all the relevant information, we aim to process your eligible claims within five working days of when we receive them.

Completing writeable pdf
You can download the latest version of Acrobat Reader here. Please note that it is generally not possible to save writeable pdf using the free Reader versions of Acrobat.

If you are using Acrobat Reader, you can use the writeable version to complete the form, but note that you will not be able to save it. When returning pdf to us, please print and sign the form before scanning and emailing or faxing.

If you are using Acrobat Pro, you can input and save the information in the pdf.  Please remember to print and sign the file before scanning and emailing or faxing it back to us.

Please note that it is not possible to save information in a pdf if you opened the file in a browser, no matter what version of Acrobat you have.
